Overview
Semen analysis is a laboratory examination of a man's ejaculate used to assess male fertility and reproductive function. It evaluates several parameters of the seminal fluid and the spermatozoa it contains, including semen volume, sperm concentration and total count, motility describing the proportion and quality of movement, morphology assessing the shape and structure of sperm, vitality, and the presence of leukocytes or abnormal forms. Additional measures such as pH, liquefaction, and viscosity provide further information about accessory gland function and sample integrity. Results are interpreted against established reference ranges to identify abnormalities such as reduced count, impaired motility, or atypical morphology, which may indicate disorders of spermatogenesis, obstruction, hormonal imbalance, or the effects of oxidative stress on sperm. Because sperm production and quality can be affected by systemic conditions, endocrine disturbances, testicular pathology, and oxidative damage to sperm mitochondria and DNA, semen analysis often complements clinical examination, hormonal testing, imaging, and, where indicated, testicular biopsy in the workup of infertility. It is also used to confirm the outcome of vasectomy and to evaluate samples for assisted reproductive technologies. As a standardized and widely available test, semen analysis is a foundational tool in andrology and the investigation of couples experiencing difficulty conceiving.
