Overview
Radiation therapy is the controlled use of ionizing radiation to eradicate or control malignant tissue by depositing energy that damages cellular DNA and blocks the proliferation of tumor cells. Delivery is broadly divided into external beam techniques, which direct photon or particle beams from outside the body, and brachytherapy, in which sealed sources are placed within or adjacent to the target, as well as stereotactic radiosurgery for precisely localized lesions. Accurate target volume definition, often guided by multimodality imaging, is central to maximizing tumor dose while sparing normal structures. The articles assembled here reflect these themes, including comparison of two- and three-dimensional brachytherapy in cervical cancer, target volume definition for radiosurgery of atypical meningiomas, and the combination of radiation with immunotherapy in melanoma. Radiation may be curative, adjuvant, or palliative, relieving symptoms such as pain and controlling local disease in carcinomas of the head and neck, cervix, and other sites. Related considerations include treatment sequencing with surgery and systemic agents, management of radiation-induced second malignancies, and radiation safety following radioiodine administration. As a core modality of oncology, radiation therapy is selected and planned according to tumor type, stage, anatomical location, and the balance between tumor control and normal-tissue toxicity.
