Overview

Diabetes diagnosis and management is the clinical process of identifying disordered glucose regulation and then controlling it to prevent acute and chronic complications. Diabetes mellitus is a group of metabolic disorders defined by sustained hyperglycemia resulting from inadequate insulin secretion, impaired insulin action, or both, with type 1 and type 2 diabetes the principal forms. Diagnosis rests on measures of glucose homeostasis, including fasting plasma glucose, the oral glucose tolerance test, glycated hemoglobin, and random glucose with symptoms, which together establish the diagnosis and characterize glycemic control over time. Management is multifaceted and individualized, combining lifestyle and nutritional measures with pharmacologic therapy and ongoing monitoring. Dietary strategy is central, encompassing carbohydrate quality and quantity, weight management, and the study of foods, nutraceuticals, and plant-derived agents investigated for effects on blood glucose. Pharmacologic options span oral glucose-lowering drugs and insulin, selected according to the type and severity of disease and the presence of complications. Continuous and self-monitoring of glucose support adjustment of therapy and reduce the risk of both hyperglycemia and hypoglycemia. Effective management also requires attention to acute metabolic emergencies and to long-term vascular, renal, retinal, and neurological complications. Research in this area addresses earlier and more accurate diagnosis, dietary and metabolic interventions, and integrated strategies to improve outcomes and quality of life.
