Overview
Acute coronary syndrome is a spectrum of conditions caused by the sudden reduction of blood flow through a coronary artery, usually when an atherosclerotic plaque ruptures and a thrombus forms over it. It encompasses unstable angina, non-ST-elevation myocardial infarction (NSTEMI), and ST-elevation myocardial infarction (STEMI), distinguished by the electrocardiogram and by biomarkers of myocardial injury such as cardiac troponin. The shared mechanism is an abrupt mismatch between myocardial oxygen demand and supply that, if unrelieved, leads to irreversible necrosis of heart muscle. Prompt recognition and reperfusion, by percutaneous coronary intervention or fibrinolysis, together with antiplatelet and anticoagulant therapy, are central to limiting infarct size and mortality. Research in this area examines the localization of the angina-related artery from the admission electrocardiogram in unstable angina and NSTEMI, and reports outcomes of percutaneous coronary intervention in elderly patients with ischaemic heart disease. Related work addresses genetic polymorphisms influencing coronary heart-disease risk, the cardiovascular effects of smoking and vaping, and renal function in the setting of acute myocardial infarction, reflecting the comorbidities and risk factors that shape prognosis.
